**All assets and training equipment are subject to availability. Region 5 Healthcare Coalition reserves the right to pull any and all equipment in at any given time. Real incidents will take precedents over trainings/exercises**

For all other requests, the Coalition will need at least a 2 weeks notice for equipment requests for training and/or non-emergent purposes.
Exceptions can be made on a case by case basis.

- Who the financial party.
- What equipment do you need to borrow.
- Quantity of each items requested.
- When do you need the equipment.
- How long will you need the equipment.
- Purpose for request of equipment.
- Location of where the equipment will be going.
- Contact person.
- 24/7phone number and e-mail address for contact person.
